SID-00454: The entry for user X was missing in the password system

I am getting "The entry for user Test2User2 was missing in the password system." for all users when I try to reset passwords.

When those same users try to login, they get, "Oops: we could not recognize you. Try again or reset your password."

The email system is not working if that makes any difference. I used to be able to create users manually however. I can't do that anymore. I have 1 older account that works fine though.

Does anyone know what is wrong?
