even though there is already a registration for that TWiki user.

Answer

I followed the instructions given by PeterThoeny in UserExistsWhenDoesNot for the passwd script - seems to have worked for me.

(In your installation try to move $query = new CGI; to inside the main function and 'my'yfy it, e.g. my $query = new CGI;. )
