SID-00253: How To Implement a Change Management Solution

TWiki could be used as as tool for a Software Change Management solution. There are plugins like ActionTrackerPlugin and WorkFlowPlugin that could help.

I was wondering how different users have implemented this and how for example the TWiki core developers have done this?

I would solve this with a tailored TWikiForms based application. The FeatureRequest application is built that way.
