SID-00584: Table with EDITTABLE - formats ??

I'm fighting with the following table.

1. the selection for the name in the second half of the table shows up in edit-mode as date-field?!
2. will it be possible, to pre-define the "radio-button" and how is pre-definition for "select"?
3. Formats from JSCalendarContrib doesn't work - see example with $d instead of $day
