SID-01086: Notes appear at the top after upgrading 4.2.3 -> 5.0.1

After following the upgrade instructions, everything appears to be fine except that on all our form pages, the notes are displayed at the top of the page rather than below the form as in the previous version.
Looking inside the files we have:
%META:TOPICINFO{author="xxxxxx" date="1248861631" format="1.1" version="1.4"}%
%META:TOPICPARENT{name="SystemInformation"}%
---+++ Notes
Blah Blah Blah
%META:FORM{name="SystemForm"}%
etc...
Is there any way to easily fix this?

A, you seem to have a view template applied to those pages. That view template is missing after upgrade. See
