Question

I'm aware that TWiki doesn't provide a way to lock attachments when editing, but I'm wondering if anyone has tried implementing it, or done any work in this area?

TWiki is working very well here, and we use attachments, but it isn't likely to be accepted as a potential "real" document management system without locking or some other means of managing edit contention.

Just looking for ideas ...

I'm not aware of any solution to this problem but I agree that 1) it is a problem and think that its 2) an important one.

Furthermore I think it could be easily fixed.

Maybe you can try having a go implementing a solution or contact one of the ConsultantsForHire. A solution should be based on DevelopBranch - I know this has a refined .lock file system. .
